RNs-evening shift 2pm to 10:30pm Evening Shift 2pm-10:30pm - Monday through Sunday Full-time Hours include every other weekend and some holidays Salary varies DOE

Duties/Responsibilities: Team Lead one to two CNA's Pass Medications Take care of residents medical, physical and emotional needs

Qualifications: Graduate from an accredited school of nursing. Must possess a valid state of NE RN license, or a temporary permit from the NE Board of Health. Minimum of two year long-term care experience preferred. Strong background in Medical-Surgical Nursing. Knowledge of gerontological theories and clinical applications of these theories. Thorough knowledge of regulatory standards governing skilled facilities preferred. Must have strong interpersonal skills. Effective verbal and written communication. Must be organized, flexible, and able to work autonomously. Candidates must not have been convicted of a crime rationally related to his/her practice involving moral turpitude.

Physical Qualifications: While preforming the duties of this position the employee must demonstrate the ability to lift fifty (50) pounds, walk, run, bend, stoop, reach, lift, push, pull, twist, crouch, crawl, balance, and climb. The employee is regularly required to use hand and arms to feel, handle objects, tools or controls. Must be able to talk, smell, hear and taste.

Specific vision/corrected vision abilities by this position includes: close, distance and peripheral vision, depth perception and the ability to adjust focus.

Job Summary: A holistic approach to the care of every resident involves a commitment to meeting their physical, medical, mental, emotional, spiritual, sexual, and safety needs. The RN is responsible for ensuring that this approach to care is taken by him/herself and all staff under his/her supervision.

Essential Functions: Utilizes the nursing process for the delivery of nursing care for each resident. Initiate and monitors all Introvenous therapies, according to Huntington Park protocol. Initiates and monitors all Naso-gastric therapy. Initiates all respiratory suctioning (LPN's may do tonsil tip suctioning.) Monitors all abdominal tubes, whether they have been placed for feeding or drainage. Re-inserts (with Physician approval) abdominal feeding tubes. Ensures the confidentiality of information in the medical record and on the nursing unit. Serves as a role model and resource for nursing personnel. Monitors the skin of all residents with actual skin break down or those at risk for skin break down. Review medical record for presence and proper completion of the "Skin Assessment Form". Monitors medical record for proper behavioral charting. Assures that all treatments are administered by staff working within their scope of practice. Assures that these forms are completed in full and remitted to the DON for filing within 48 hours of the error. Directs medication error forms made by temporary staff to the DON. Reviews medication administration records on a weekly basis for accuracy. Reviews medication administration records for proper staff initial and name notation. Assures that the PRN medication administration record reflects documentation for the use of all PRN medications and the response of the resident to these interventions. Assures that the daily medication administration record reflects the residents refusal of medications and the response of the resident to these interventions. Monitors the medication administration process on a weekly basis. This assessment will assure that the residents are given 2 CAL supplement with their medications (if appropriate). Only appropriate oral medications are crushed, and all medications are administered with an acceptable liquid. All medications are administered by "tube" are appropriately crushed and given one at a time and followed by the proper flush, and by an LPN or RN. Reviews list of outdated or discontinued drugs prepared by night shift nurse, for accuracy before faxing to the pharmacist. Contacts the pharmacist if the outdated or discontinued drugs reappear on the medication administration record. Monitors all injectable medications for out dates weekly. Monitors all injectable medications for "date opened" and notation on bottles. Monitors the narcotic sign-out form on a weekly basis (more often if necessary). Reports any inappropriate entry or count (missing drugs) immediately to the DON (or administrator in absence of the DON). Monitors the use of psychotropics, alert to need for dosage increase/decrease, or order to discontinue. Directs the maintenance, cleaning, and stocking of the medication carts according to the cleaning schedule. The carts are to be cleaned alternatively (weekly), by the day and evening CSM's. Monitors ADL forms on a daily basis for compliance by the CNA's. Monitors all lab and X-Ray results. Addresses all Infection Control issues, reports to Infection Control nurse as warranted. Assesses all residents with a recognized or reported change of status. Monitors "hydration" form. Monitors the repositioning of all residents who sit in wheelchairs for long periods or are bedfast,assures position change for at least every two hours. Directs referrals to other departments as necessary. Monitors shift reports for pertinence and accuracy. Investigates all incident reports on the nursing unit (during his/her shift) and assures proper-up. Delivers these completed reports to the Medical Records Coordinator. Initiates Physician appointments, medical and dental, hospital X-Ray appointments, therapy interventions, hair and other personal appointments as warranted. Monitors the van schedule on a daily basis, reminding staff to have the residents properly attired, toileted and ready for all appointments. Assures that all teeth and eyeglasses are marked for identification. Creates and maintains a safe working environment. Reports maintenance needs for unsanitary conditions to appropriate departments for intervention. Directs staff and acts as resource person for disaster drills or actual events. Takes care of residents medical, physical and emotional needs.
